New patient registration

If you live within our practice area you are welcome to register with us and our reception team will be happy to guide you through the procedure. New patients will be asked to complete a simple health questionnaire and under some circumstances may be asked to attend an appointment with the nurse.

Please call in to the Surgery to collect a Registration Form and New Patient Questionnaire.

Updating your contact details

If you need to change name, address, telephone number (home or mobile) or e-mail address please let our receptionists know by calling or writing to us. Alternatively you can inform us of your new address/phone number/e-mail by completing an on-line form.

If you move outside the practice area you will need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.

New Patient Registration

DURING CORONAVIRUS (COVID-19) PERIOD

We welcome new patients.

We require all new patients to complete a registration form (GMS1W), and a health questionnaire (below) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be found below.  We will accept two forms of identification that prove you live within the area, photo and one with proof of address. Your NHS number is also required to register, this can be obtained from your previous surgery. Please also supply a copy of any repeat medications from your previous GP to ensure we have proof of medication. 

Please complete and attach using the online forms below. We will accept a photograph of the evidence; if you can not access a printer/scanner. Please note if you do not complete all information required or attach proof of address this will delay your registration.

For patient safety and confidentiality; please note by returning a completed form you are signifying you are who you say you are and all information is correct to the best of your knowledge. This will mean you also give permission for us to set you up as a New Permanent Patient at our Practice.
